This page explains, in plain language, what personal information Phoenix Sports Academy collects about families in our junior programs, how we use it, and the choices you have. If anything here is unclear, email us and we will talk it through.

What we collect at registration

When you register a child, we collect the parent or guardian's name, email address and phone number, and the child's first name, last initial, date of birth, training location and sport or sports. The registration form is submitted by email through a third-party form service (Web3Forms) and arrives in the academy's email inbox. In this phase of the site it is not stored in a database.

The registration form includes a consent checkbox. Our lawful basis for displaying a child's name and assessment results on the public site is the consent you give there as the parent or guardian.

What we publish on this website

For each player we publish only their first name and last initial, age group, training location, sport or sports, and their skill and fitness assessment results over time. Where two players share the same first name and last initial, we may show a surname so families can tell them apart.

We do not publish a child's full date of birth, home address, parent contact details, or photographs. Photographs are only ever published with separate, specific parental consent.

Assessment data

Coaches record skill and fitness assessments during the testing blocks that run every two months. This data is used to show each family their child's progress, and to rank performance within age groups on the site's leaderboard.

Who your information is shared with

We do not sell personal information and we do not share it for advertising. The only third parties involved are the form-email provider (Web3Forms) and the website host (Firebase Hosting, provided by Google), and both are used solely to operate this site.

How long we keep it

Registration emails and assessment records are kept for as long as the child is enrolled in the program, and are removed on request.

Your choices

As a parent or guardian you can ask to see, correct, or remove your child's information, or withdraw consent for public display, by emailing phoenixsportsacademysc@gmail.com. Removal is actioned on the next update to the site.
